Forms
The State of Virginia requires that each student have on file a Commonwealth of Virginia School Entrance Health Form, which includes health information, a physical exam report, and a certificate of immunization. Additionally, Green Hedges requires every student to submit an emergency record and physical exam each year. Pertinent health and emergency information should be updated as necessary. Students will not be permitted to participate in recess or sports unless their physical examination, emergency release, and permission forms are on file with the school. Forms for all students must be completed by August 15. Please be certain to fill in all information requested including all names and telephone numbers.

There are several forms/requirements that need to be downloaded and returned to the School.
Physical Examination Form – for all returning students. This form needs to be completed by your child’s physician and returned by August 15, 2011.
Commonwealth of Virginia School Entrance Health Form – for new students only. This includes immunization records as well as a physical exam by your child’s doctor. The form needs to be completed by your child’s physician and returned by August 15, 2011.
The Authorization for Administering Medicine Form – for any student requiring administration of medicine during school hours. If your child requires medication during school hours this form needs to be completed, signed by a licenced prescriber, and returned to Green Hedges by August 15, 2011.
School & Day Care Minimum Immunization Requirements
Proof of Identity for new students must be provided to Green Hedges prior to August 15, 2011.
Acceptable Evidence of Immunity to Varicella Virus – for new students only (proof of vaccine for Chicken Pox or confirmation of having had Chicken Pox)
(Tdap) booster shot – Per order of the Virginia General Assembly, all Rising Grade 6 students must have a tetanus, diphtheria, pertussis prior to entry into school this fall if at least five years has passed since the last shot.
